IMPORTANT NOTE REGARDING NEW YORK FORWARD PHASE II RE-OPENING & the DEPARTMENT OF CONSUMER AFFAIRS
During the New York Forward Phase II Re-Opening, in order to limit any future spread of COVID-19, the Department of Consumer Affairs will reopen to the public BY APPOINTMENT ONLY.
Appointments may be scheduled by emailing consumeraffairs@nassaucountyny.gov please include in the subject line: “Schedule an Appointment.”

Renewal applications for existing licensees are mailed three months ahead of the license expiration date (this is a new practice starting January 2020). If you would like a fillable PDF of your renewal application sent to you, please request through consumeraffairs@nassaucountyny.gov please include in the subject line “Email Renewal Application.”
We thank you for your patience; these changes are designed with our public health as the first and foremost consideration. Please remember, if you are not feeling well, stay home.
All pre-hearing settlement conferences will be automatically adjourned. You will receive a notification regarding your adjourn date. Any documents than need to be submitted to the Department of Consumer Affairs can be scanned and emailed to: consumeraffairs@nassaucountyny.gov.

Fingerprinting Procedure:
Mail your completed application and proper payment to the
Office of Consumer Affairs, 240 Old Country Road, Mineola, NY 11501.
Once your application has been received and reviewed, a staff member will contact you to set up an appointment to be fingerprinted.
Complaints Must Be In Writing: All complaints must be received in writing, signed and dated. CLICK HERE to download and print a complaint form. Once the form and backup documentation have been received, your case will be assigned to an investigator for review and appropriate action.
